Privacy policy (this website)

This policy describes how Vantara Dental, PLLC ("we," "our," "the practice") collects and uses information through vantaradental.com. It is separate from — and does not replace — the HIPAA Notice of Privacy Practices below, which governs your protected health information inside the practice.

What we collect on the website

  • Information you provide. Name, phone, email, and any message content you submit through our contact or appointment-request forms.
  • Automatic data. IP address, browser type, pages visited, referring URL, time-on-page, and similar analytics signals.
  • Cookies. We use first-party cookies to remember form preferences and privacy choices, and a small set of analytics cookies (described below) that you can decline.

How we use this information

  • To contact you about an appointment, membership, or request you initiated.
  • To improve our website — which pages are helpful, which are confusing, which are broken.
  • To meet legal, regulatory, or quality-of-care obligations.

We do not sell, rent, or trade your information. We do not run ad-targeting pixels that share protected health details with third parties.

Analytics & third-party services

We use a privacy-conscious analytics tool to understand aggregate usage. No individual health information is sent to analytics. We use the following vendors:

  • Google Fonts — to render typography (fonts.googleapis.com).
  • Pexels — to serve certain editorial imagery.
  • Formspree — to receive appointment-request submissions (TLS 1.3, encrypted-at-rest).
  • PostHog / Google Analytics 4 — aggregate traffic analysis, IP-anonymized.

Your choices

  • Decline non-essential cookies via the consent banner on your first visit.
  • Ask us to delete any data you've submitted through our website by emailing privacy@vantaradental.com.
  • Use your browser's Do-Not-Track signal — we honor it for analytics cookies.

Children

This website is not directed to children under 13. We do not knowingly collect information from children online. Parents who believe we may have collected information from their child should contact our Privacy Officer so we can delete it.

HIPAA Notice of Privacy Practices

Effective date: February 1, 2026. This notice describes how medical information about you may be used and disclosed, and how you can access this information. Please review carefully.

Our commitments to you

  • We are required by law to maintain the privacy and security of your protected health information (PHI).
  • We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you tell us we can, in writing.

How we typically use or share your health information

  • Treatment. We use your information to provide dental care and to coordinate with other providers you authorize.
  • Payment. We may share information with your insurance carrier to obtain payment for services.
  • Operations. We may use it to run the practice, improve care, and contact you about appointments.
  • Required by law. We share information when required by state or federal law, court order, or public-health authority.

Uses and disclosures that require your written authorization

  • Marketing purposes outside of our practice communications.
  • Sale of any health information.
  • Most sharing of psychotherapy notes.
  • Fundraising communications (and you may opt out at any time).

Your rights

  • Get an electronic or paper copy of your record, usually within 30 days.
  • Ask us to correct your record if you think it is wrong or incomplete.
  • Request confidential communications — for example, email only, or text at a specific number.
  • Ask us to limit what we use or share for treatment, payment, or operations (we are not always required to agree).
  • Get a list of disclosures we've made of your information in the past six years.
  • Choose someone to act for you — a legal guardian or medical power of attorney.
  • File a complaint if you feel your rights have been violated — with us or with the U.S. Department of Health and Human Services Office for Civil Rights. No retaliation, ever.

Changes to this notice

We may change this notice. Changes apply to all information we have about you. The new notice will be available on request, in our office, and on this page.

Accessibility statement

Vantara Dental is committed to providing a website that is accessible to the widest possible audience, including people with disabilities. We aim to conform to the Web Content Accessibility Guidelines (WCAG) 2.2, Level AA.

What we do

  • Semantic HTML, accessible landmarks, and skip-navigation links.
  • Color contrast that meets or exceeds 4.5:1 for body text.
  • Full keyboard navigation, visible focus states, and labeled form fields.
  • Alt text on every informational image; decorative images marked as such.
  • Audio and video (when used) captioned and transcribed.

In our office

  • Elevator access from the parking garage and street level.
  • ADA-compliant restroom and accessible check-in desk.
  • Hoyer-compatible treatment chair for patients with mobility needs — please let us know 24+ hours before your visit.
  • ASL interpreter available on request; please give us 5 business days' notice when possible.
  • Large-print intake forms and high-contrast patient-education materials on request.

If you encounter an accessibility barrier on this website or in our office, please tell us. Email access@vantaradental.com or call (512) 555-8242. We typically respond within 2 business days and resolve reported issues within 30 days.

Good Faith Estimate (No Surprises Act)

Under the federal No Surprises Act, healthcare providers must give uninsured and self-pay patients an estimate of expected charges before a scheduled service.

Your right to a Good Faith Estimate

  • You have the right to receive a written Good Faith Estimate of the total expected cost of any non-emergency dental item or service from us.
  • Make sure your provider gives you a Good Faith Estimate in writing at least 1 business day before your service. You can also ask for one before scheduling.
  • If you receive a bill that's at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Keep a copy of the estimate in a safe place.

How we provide estimates at Vantara Dental

Every treatment plan we send — whether you have insurance or not — includes an itemized estimate of fees, your expected out-of-pocket, and an explanation of what the numbers include and exclude. For self-pay patients, that document is your Good Faith Estimate.
